What is Metronidazole (Flagyl)?

Metronidazole is the active ingredient in the prescription medicine Flagyl. Flagyl is commonly used to treat various types of bacterial and parasitic infections in different areas of the body.

It belongs to the class of medicines known as antibiotics and works by killing bacteria or stopping their growth. It’s effective in treating infections caused by bacteria that do not need oxygen to survive (anaerobic bacteria).

Flagyl ER and Likmez are other different brand names for the active ingredient and generic medicine, Metronidazole.

What is Metronidazole used for?

Metronidazole is often prescribed to treat infections of the:

  • Genital tract and pelvic area
  • Digestive tract (stomach and intestines)
  • Skin, bones and joints
  • Gums and other dental infections

Some conditions this medicine is prescribed for include trichomoniasis (an STI), amebiasis, endometritis, endocarditis and more.

How does Flagyl 500mg work to treat BV?

Metronidazole, the active ingredient in Flagyl, works by stopping the bacteria from growing. When bacteria cells enter your body, they want to make copies of themselves to spread.

After taking Metronidazole, it enters the bacteria's cell and disrupts its DNA. It stops bacteria from making new DNA and breaks down their existing DNA, which ultimately causes the bacteria to stop growing and die.

The healthy balance of bacteria returns to normal, relieving the symptoms of BV.

What Metronidazole dosages are available and which one is right?

This medicine is generally available in a range of dosage forms, such as:

  • Capsules
  • Tablets (immediate-release and extended-release)
  • Oral suspension
  • IV solution
  • Vaginal gel

Within these forms, this medication is available in a variety of strengths. In tablet form, this medication is available as either 250mg or 500mg tablets. The dose you are prescribed depends on the condition being treated.

To treat BV, the usual adult dose of metronidazole is 500mg taken orally twice daily for 7 days.

How should Metronidazole (Flagyl) be taken?

For BV, take 1 tablet (500mg) twice a day for 7 days. Space out this medicine by at least 8 hours, try to take one tablet in the morning and one tablet at night.

Swallow the tablets whole with a glass of water, either during or just after you've eaten some food. Do not crush or chew the tablets.

If you forget to take your dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and take the next dose at the right time. Do not double the dose.

How long does it take for Metronidazole to work?

Metronidazole is quickly absorbed and starts to work a few hours after you take it. Once you start taking the medicine, your symptoms might go away within a couple of days. However, you should continue taking this medication until you have finished the course, even if you feel better. If you stop your treatment early, the infection could come back.

Who can and cannot use Metronidazole?

Most people can take Metronidazole safely. Do not take this medication if:

  • You are allergic to Metronidazole or any of the ingredients present in this medication
  • You have taken a medicine called Disulfiram (used for alcoholism) in the past 2 weeks
  • If you have a condition known as Cockayne syndrome

To see if this medication is suitable for you, speak to your doctor if you:

  • Have liver disease
  • Have kidney disease
  • Have blood disorders
  • Have problems with your brain, spinal cord or nerves
  • Are taking any other medicines

Is Metronidazole safe in pregnancy?

Studies have shown that Metronidazole can be taken to treat BV during pregnancy. However, if you are pregnant or breastfeeding, check with your doctor before using this medication.

What are the possible interactions?

This medicine may interact with other drugs you take. This includes any over-the-counter medicines, herbal products, and supplements.

Some of Metronidazole's interactions include:

  • Alcohol (including some cough mixtures)
  • Disulfiram (Antabuse)
  • Warfarin or other blood-thinning medicines
  • Lithium
  • Epilepsy treatments (e.g. Phenytoin)
  • Cancer treatments including Busulfan
  • Ciclosporin (taken after an organ transplant)

Metronidazole and alcohol

Avoid alcohol while you are taking this medication and for at least and for at least 3 days after stopping it. Drinking alcohol with metronidazole may make you feel sick, vomit, and have stomach cramps, headaches and flushing.

What are some side effects of Metronidazole?

As with all prescription medicines, this medicine may cause some side effects. These are usually mild and will go away by themselves.

Some of the most common side effects include:

  • Nausea
  • Headache
  • Loss of appetite
  • Metallic taste in the mouth
  • Stomach upset (diarrhea or constipation)
  • Stomach cramps

If these side effects bother you, contact your doctor.

Some side effects can be serious. If you experience any of the following symptoms, seek medical care immediately:

  • Numbness, pain, burning, or tingling in your hands or feet
  • Seizures
  • Sudden vision changes
  • Problems with coordination and/or confusion

Some people may be allergic to certain antibiotics. If you experience signs of an allergic reaction, stop taking this medication and get medical help immediately.

  • Skin rash or hives
  • Itching
  • Swelling of your lips, face, or mouth
  • Flushing
